Isoproterenol-induced elevated bispectral indexes while undergoing radiofrequency ablation: a case report.

چکیده

The use of bispectral index (BIS) for monitoring neuronal function under general anesthesia has been expanding in practice. However, the meanings of the values are not always clear and have been shown to be altered by such drugs as ketamine, ephedrine, and physostigmine. Presented here is a case of increasing BIS in response to noradrenergic beta receptor stimulation via the administration of isoproterenol while under general anesthesia. The rise in BIS values appeared to have no correlation to the stimulation produced by the procedure. The patient reported no intraoperative recall despite BIS values in the high 70s. This case reinforces that BIS values require interpretation and can only be relevant to the circumstances of the individual case.

Pulmonary Embolism as a Complication after radiofrequency catheter ablation Study: A Case Report

Pulmonary thromboembolism (PTE) is a fatal condition that may rarely occur due to complications of coronary catheter insertion. In this case report, a 41-year-old male was presented 48 hours after radiofrequency catheter ablation(RFCA) for the management of Wolf-Parkinson-White syndromewith acute onset of dyspnea, hemoptysis, and chest pain. The physical examination revealed coarse crackles in ...

Clinical and electrophysiological features of respiratory cycle–dependent atrial tachycardia: An analysis of three cases

Introduction Focal atrial tachycardia (AT) accounts for 5%–15% of arrhythmias in adults who undergo electrophysiological study (EPS) for paroxysmal supraventricular tachycardia, and it can be generated by enhanced automaticity, triggered activity, and microreentry. Radiofrequency catheter ablation (RFCA) has been reported as a curative therapy for AT with a high success rate. While almost all i...

Improvement of left ventricular function in patients with persistent atrial tachyarrhythmia induced cardiomyopathy undergoing radiofrequency ablation

Purpose: To investigate the alteration of left ventricular function in subjects with persistent atrial tachyarrhythmia induced cardiomyopathy (TIC) undergoing radiofrequency ablation, and to study the pathogenesis and effective treatment of TIC. Methods: A total of 25 cases with persistent atrial tachyarrhythmia and impaired left ventricular systolic function were studied (16 men and 9 women, a...

Combined Right Ventricular Outflow Tract Epicardial and Endocardial Late Potential Ablation for Treatment of Brugada Storm: A Case Report and Review of the Literature

A 34-year-old man with Brugada syndrome (BrS) presented with electrical storm, manifested as multiple appropriate shocks from his implantable cardioverter-defibrillator over a period of 7 hours. He had not tolerated prior treatment with quinidine, and had self-discontinued cilostazol citing persistent palpitations. After stabilization with intravenous isoproterenol, an electrophysiology study w...

Multiple Arrhythmogenic Substrate for Tachycardia in a Patient with Frequent Palpitations

We report a 26-year-old woman with frequent episodes of palpitation and dizziness. Resting electrocardiography showed no evidence of ventricular preexcitation. During electrophysiologic study, a concealed right posteroseptal accessory pathway was detected and orthodromic atrioventricular reentrant tachycardia incorporating this pathway as a retrograde limb was reproducibly induced. After succes...
